Complex Case Management: Multi-System Coordination trains Canadian counsellors and healthcare professionals in collaborative, trauma-informed, culturally safe approaches to coordinate care across mental health, primary care, justice, housing and social services. Participants learn interagency negotiation, care planning, risk stratification, system navigation and documentation techniques to improve client retention, reduce service gaps and produce measurable multi-sector care plans.

What you'll learn

Implement trauma-informed, Indigenous cultural safety and anti-racism principles when designing and coordinating multi-sector care plans in Canadian settings.
Negotiate, document and operationalize interagency care pathways across mental health, primary care, justice, housing and social services while adhering to Canadian privacy and consent frameworks (e.g., PHIPA, PIPEDA, provincial acts).
Apply risk-stratification, social-determinants screening and measurement frameworks to create measurable multi-sector care plans that improve retention and reduce service gaps.

Requirements

  • Current practice as a counsellor, social worker, nurse, case manager, allied health professional, or equivalent role in Canada.
  • Foundational knowledge of mental health, basic case management, and local service systems (or completion of an introductory case management course).
